TransMedics Group is building a vertically integrated organ transplant logistics network that could make it the dominant infrastructure provider in the U.S. and Europe within a decade. The company's Organ Care System keeps donor organs alive outside the body, and its National OCS Program operates a fleet of 22 aircraft with clinical teams to transport organs. A kidney preservation program in development would dramatically expand its addressable market, given over 90,000 Americans are waiting for kidney transplants. TransMedics is also scaling in Europe through an investment in German aviation operator PAD Aviation and a ground transport collaboration in Italy. While heavy spending on expansion compressed margins and contributed to a 45% share price decline over six months, revenue still grew 21% year over year in the first quarter of 2026.
